Caspofungin acetate is an antifungal medication that treats a variety of fungal infections. It is used to treat serious fungal infections, including candidemia (fungal infection in the blood), esophageal candidiasis (fungal infection of the esophagus), other candida infections, and aspergillosis (fungal infection in the lungs). Caspofungin acetate was approved by the US Food and Drug Administration (FDA) and the European Medicines Agency (EMA) in 2001. Caspofungin acetate, also known as CANCIDAS, is synthesized from pneumocandin B0, a fermentation product of the fungus Glarea lozoyensis. The synthesis process involves chemical modifications at two sites of the peptide core, including: Reduction of a primary amide to an amine and Condensation of the hemiaminal moiety with ethylenediamine. In the 263 patients, given caspofungin in randomized double-blind active-control trials to date, there have been no serious clinical or laboratory drug-related adverse events; caspofungin was discontinued in only 2% of these patients because of drug-related adverse experiences. Caspofungin may have potentially important drug interactions with cyclosporine and tacrolimus.

Caspofungin has activity against Aspergillus spp. as well as a variety of Candida spp. Clinical data support its usefulness in the treatment of invasive aspergillosis and select candida infections. As additional clinical data become available, it seems likely that the therapeutic role of caspofungin will expand.
